Monkey's Tail LP Reservation Request
 Please fill out the below form and someone will be in touch to confirm your reservation. If you do not receive a confirmation email from an employee about your party, your reservation is NOT confirmed.
 This reservation request is to have space/table(s) reserved for your large party, but your group will still have to order at the bar. We do not have servers. 
  We do not charge if you would to bring any cakes or pastries for your event, but we may not be able to accommodate refrigeration space. Decorations at your table(s) are welcome. Glitter and/or confetti(including inside balloons) are not allowed. Please plan accordingly.
**Please be aware that no one under 21 years of age is allowed after 8pm every day of the week and subject to change according to bar events.
**Reservations are only for large parties of 10 or more.   
**We do NOT accept reservations for Cinco de Mayo and other holidays.
**SAME DAY RESERVATIONS MAY NOT BE ACCEPTED.
