Use this form to request library instruction for your course(s), bearing in mind that sessions are most
effective when:

• The purpose of the library instruction directly relates to a class assignment or project.
• You, the instructor, accompany the class to each library instruction session.
• Instruction requests are submitted at least ONE WEEK in advance of the preferred date of the
session.


Please include a description of the assignment your students will be working on. Include information
about what you would like your students to be able do as a result of the session to help us create a more
useful session for your students.
