Deklaruję świadomość konieczności regularnych wpłat składek członkowskich zgodnie z §13 statutu. Brak wpłaty składek w terminie może spowodować utratę członkostwa. Zgodnie z przyjętą uchwałą na ostatnim Walnym Zebraniu Członków (10.07.2021) składki wynoszą: 120 zł rocznie - dla Członków Stowarzyszenia posiadających aktywny status studenta lub ucznia155 zł rocznie  - dla Członków Stowarzyszenia niebędących studentami/I declare that I am aware of the necessity of regular payments of membership fees in accordance with §13 of the statute. Failure to pay contributions on time may result in loss of membership. In accordance with the resolution adopted at the last General Meeting of Members (10/07/2021), the fees amount to: PLN 120 per year - for Association Members with active student or student status, PLN 155 per year - for Association Members who are not students *
Informacyjna Klauzula RODO/GDPR Clause
Zgodnie z art. 13 ogólnego rozporządzenia o ochronie danych osobowych z dnia 27
kwietnia 2016 roku (dalej „RODO”) informujemy, że:
1. Administratorem danych osobowych jest United Nations Association Poland, ul.
Kobierzyńska 117A/8, 30-382 Warszawa, KRS 0000395887, biuro@unapoland.org
(dalej „Stowarzyszenie”).
2. Dane osobowe przetwarzane są wyłącznie w celu przeprowadzenia rekrutacji oraz
uczestnictwa w pracach Stowarzyszenia mających na celu realizacje jego celów
statutowych. Podstawą do przetwarzania danych osobowych jest dobrowolna zgoda na
przetwarzanie danych.
3. Zgodnie z art. 13 ust. 2 pkt. A RODO okres przechowywania danych osobowych upływa w
momencie ustania członkostwa w Stowarzyszeniu.
4. Dane osobowe będą przetwarzane w sposób zautomatyzowany jak i niezautomatyzowany.
5. Przysługuje Pani/Panu prawo do:
- dostępu do swoich danych osobowych oraz otrzymania ich kopii
- sprostowania swoich danych osobowych
- usunięcia swoich danych osobowych
- wniesienia sprzeciwu przeciwko przetwarzaniu danych osobowych
- przeniesienia danych osobowych
- wniesienia skargi do Prezesa Urzędu Ochrony Danych Osobowych
Ponadto uprzejmie informujemy, iż przysługuje Pani/Panu prawo do wycofania zgody na
przetwarzanie danych osobowych oraz prawo do żądania od administratora danych osobowych
niezwłocznego usunięcia danych osobowych w przypadku cofnięcia członkostwa – na zasadach
określonych w artykule 17 Rozporządzenia Parlamentu Europejskiego i Rady UE z dnia 27
kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych
osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy
95/46/WE. Cofnięcie zgody nie będzie miało wpływu na przetwarzanie dokonane przed jej
wycofaniem.

Pursuant to Art. 13 of the general regulation on the protection of personal data of 27
April 2016 (hereinafter referred to as "GDPR"), we would like to inform you that:

Kobierzyńska 117A / 8, 30-382 Warsaw, KRS 0000395887, biuro@unapoland.org
(hereinafter the "Association").
2. Personal data is processed only for the purpose of recruitment and
participation in the work of the Association aimed at achieving its goals
statutory. The basis for the processing of personal data is the voluntary consent to
data processing.
3. Pursuant to Art. 13 sec. 2 points A GDPR, the period of personal data storage ends in
upon termination of membership in the Association.
4. Personal data will be processed in an automated and non-automated manner.
5. You have the right to:
- access to your personal data and receipt of a copy thereof
- rectify your personal data
- delete your personal data
- objecting to the processing of personal data
- transfer of personal data
- lodging a complaint to the President of the Personal Data Protection Office
In addition, we kindly inform you that you have the right to withdraw your consent to
processing of personal data and the right to request the administrator of personal data
immediate deletion of personal data in the event of membership withdrawal - on the terms
specified in Article 17 of the Regulation of the European Parliament and the EU Council of 27
April 2016 on the protection of individuals with regard to data processing
personal data and on the free movement of such data, and repealing the Directive
95/46 / EC. Withdrawal of consent will not affect the processing carried out before it
withdrawal.
