Only fill out this form if you DO NOT have internet that your student or students can access. Please complete one form for each of the DCS students that live in the household at the address you will enter below. ( For Example: If your family has 5 students that attend DCS and they all live at the same address we will need one form entry per student.) There is a permission letter that must be signed before the hot spot can be sent home. The letter can be downloaded from the website and returned to the school signed. If you can't print it one can be provided to the student.
