2. You are allowing the organisers to contact you using the information you have provided for any issues that may arise pertaining to the 34th Philippine Chemistry Congress.

4. Attendance records and your PRC license number will be forwarded to the Professional Regulation Commission (PRC) for the awarding of CPD units.
