PLEASE READ BEFORE COMPLETING THIS FORM. Students must have completed 6th-12th grades to participate. Students must sign up with a church that has an adult leader staying the night. STUDENTS ARE REQUIRED TO STAY AT CAMP THE DURATION, IF A STUDENT LEAVES THEY WILL NOT BE PERMITTED TO RETURN.
During camp check-in, students and parents will receive a printed form to sign. PARENTS MUST COME IN TO CHECK IN AND SIGN THE FORM. Cost is $20 due when you check in to camp. Please give registration fee to your youth leader.
