Sample Reflection Template
We use a form like this to gather weekly reflections from educators.  Teachers are invited to type directly into this form, cut-and-paste from a word processing document, or email us a picture/scan of handwritten notes.  

We typically gather this information on Thursday afternoons, then review and report it back to teacher teams on Fridays.  This enables us to highlight effective practices, address emergent challenges, and modify curriculum and professional learning as necessary to support each district's instructional team.
