Please use this form to make a club order of face masks.  There is no minimum order.  Each face mask is £3.50 + P&P.  Profits will be going to Rotary Foundation.Once your form has been submitted you will receive an emailed copy.  Please check this and let me know ASAP if there are any errors.  Once your order has been received you will be emailed with payment details and the amount you should pay.  We aim to complete order within 2 weeks.
