CoS attendees are encouraged to use this form (one form per person) to learn more about ways to be involved in CoS ministries and indicate interest in serving.  You only need to check the boxes for the areas you have interest in.  You are not required to respond to any of the opportunities, and can submit the form once you have gone through all of the questions, even if you have no responses.
