By joining the whitelist for the Diagnostic Network for Secondary Hypertension (DNeSH), you agree to the terms and conditions outlined in this document. The Project is managed by André FR Silva on behalf of the Sociedade Portuguesa de Hipertensão, based in Lisbon, Portugal. The purpose of collecting your personal and professional data is solely to manage the whitelist of doctors eligible to participate in the Project and facilitate the submission of anonymized clinical cases.

All data will be processed in compliance with the General Data Protection Regulation (GDPR) (EU Regulation 2016/679).

The personal and professional information provided will be used exclusively for:

1. Verifying eligibility to participate in the Project.
2. Communicating with you regarding updates, case submissions, and Project-related activities.
3. Managing the whitelist of doctors authorized to submit clinical cases to the DNeSH database.

The processing of your personal data is based on your explicit consent, which is given by completing and submitting the whitelist application form. You may withdraw your consent at any time by contacting the Data Controller through the SPH website.

Your data will be retained for the duration of the DNeSH Project or until you request its deletion, whichever comes first. After the Project concludes, all personal data will be securely deleted unless further retention is required by applicable law.

The Sociedade Portuguesa de Hipertensão will not share your personal data with third parties without your explicit consent unless legally obligated to do so.

Under GDPR, you have the following rights concerning your personal data:

• Right of Access: You can request access to the personal data we hold about you.
• Right of Rectification: You can ask for corrections to any inaccurate or incomplete data.

When contributing clinical cases to the DNeSH database, the following conditions must be met:

1. Patient Consent:

   • Each submitted clinical case must be accompanied by the explicit consent of the patient. Consent must be obtained using the official Patient Consent Form available on our website.
   • The form must be signed by the patient, and the original copy should be securely stored by the submitting doctor. While the signed form does not need to be uploaded, it must be made available upon request for audit purposes.

2. Anonymization:

   • All patient data submitted to the DNeSH database must be fully anonymized to ensure compliance with GDPR and ethical standards.
   • Do not include any personally identifiable information (e.g., name, date of birth, address, or contact details). Only anonymized clinical information relevant to the diagnosis and management of secondary hypertension may be submitted.

3. Compliance with Local Regulations:

   • You must ensure that all submissions comply with the data protection and medical regulations of your local jurisdiction in addition to the GDPR requirements.

4. Accuracy and Integrity of Data:

   • You are responsible for ensuring the accuracy, completeness, and integrity of the submitted clinical data. Submissions found to contain misleading or false information may result in removal from the whitelist.

The Sociedade Portuguesa de Hipertensão reserves the right to modify these terms and conditions at any time. Any significant changes will be communicated to whitelist participants in advance.
