The purpose of collecting your personal and professional data is solely to manage the whitelist of doctors eligible to participate in the Project and facilitate the submission of anonymized clinical cases.

8. Case Submission RequirementsWhen contributing clinical cases to the DNeSH database, the following conditions must be met:

- Each submitted clinical case must be accompanied by the explicit consent of the patient. Consent must be obtained using the official Patient Consent Form available on our website.
- The form must be signed by the patient, and the original copy should be securely stored by the submitting doctor. While the signed form does not need to be uploaded, it must be made available upon request for audit purposes.
Anonymization:
- All patient data submitted to the DNeSH database must be fully anonymized to ensure compliance with GDPR and ethical standards.
- Do not include any personally identifiable information (e.g., name, date of birth, address, or contact details). Only anonymized clinical information relevant to the diagnosis and management of secondary hypertension may be submitted.
Compliance with Local Regulations:
- You must ensure that all submissions comply with the data protection and medical regulations of your local jurisdiction in addition to the GDPR requirements.
Accuracy and Integrity of Data:
- You are responsible for ensuring the accuracy, completeness, and integrity of the submitted clinical data. Submissions found to contain misleading or false information may result in removal from the whitelist.
