Please join us in one of the areas largest soft play areas which we have hired out just for our church groups from 6:15-7:45pm on Friday 21st November 2021 at Activity World, Padholme Road East,

Parking is available and parents are welcome to stay. Refreshements are available to buy, though please feel free to send some water with your child too.

I have read the attached information and give permission for my child to take part in the Activity World trip on the 19th November 2021

I give permission for the attic cafe leaders to administer first aid in case of an emergency.

I give permission for the attic cafe leaders to sign on my behalf, any written form consent required by the hospital authorities should any emergency treatment be necessary and provided that the delay required to obtain my signature might be considered, in the opinion of the doctor or surgeon concerned, to be likely to endanger my child’s health and safety.

I give permission for my child to be given paracetamol if it should be needed unless it has been stated that the young person is unable to take it.

I opt in and am happy for this information to be used to keep my child informed about future St. Neots Evangelical Church youth / children’s events during this academic year. I understand that my child’s information will be securely stored on paper for 1 year from the above date for the church’s records. This information will not be passed to any other party without expressed consent from the individual unless the church are legally obliged to do so.
