Please agree to the following: I can confirm my child is currently in Year 6 and is on roll at. We understand that all families are offered one visit to Erasmus Darwin Academy this term and there is no capacity for an additional visit. GDPR notice: The Academy will store your information for three weeks after your visit in the event that we are required to provide details for NHS Track and Trace. We would also request that all adults complete a Lateral Flow Test and do not attend the Academy in the event that this indicates a positive result. I understand that visits will not be permitted for anyone (including students) who displays formal symptoms of COVID-19 (which are a new continuous cough, fever or loss of taste/smell). I will not attend if I, or my child, are subject to self isolation, as directed by NHS Track and Trace. *
