PD Day Registration Form 2019-20
~All PD Day programs will take place at our Meredith Centre location, 23 Cecil Rd, Chelsea, QC.  
~All programs require a minimum of 4 participants to run. If we do not have the minimum number registered 48  hours in advance of the program, the program will be cancelled.
~Please note that this is a school age program. Children must be 5 years of age or older  ie. in at least Senior Kindergarten to participate.
~If you are registering more than one child, please create a separate registration form for each of your children. This will help to provide us with critical information such as medical details pertaining specifically to each child in order to keep everyone safe. It also helps us to accurately account for the number of students registered since each form filled out counts as one registrant only. I understand that due to the nature of Forest School, my child must come properly prepared, with appropriate clothing and additional supplies. These requirements will be outlined in the information received following registration. *
I understand that I may be called to pick up my child if he/she is not properly prepared/dressed and this is creating a risk to the safety of my child or to others.

Photos and videos will be taken of the children for use in our promotional materials. These may include but are not limited to Facebook and other social media sites, posters, newspaper articles and pamphlets. Please indicate below whether you give permission for your child's photo to be included in these materials.

Although we do everything we can to prevent injury, Forest School carries with it a certain level of risk.  This is an outdoor program and nature is seldom predictable. At Forest School we encourage the children to take supported risks. In this way, they learn how to manage risk in a responsible manner. Strict safety procedures are in place and injuries are most likely to occur if these procedures are not followed. Risk/benefit assessments are completed on every activity and reviewed regularly. We have low child- adult ratios and all tool use is supervised closely. Tool use is only permitted after proper instruction and when the Forest School Educator believes the child is ready. There is the risk of injury that includes, but is not limited to, cuts, scrapes, bruises, insect bites, poison ivy as well as more serious injuries. I acknowledge that Forest School carries with it an element of risk and the possibility of injury . I understand that injuries sustained during outdoor activities have the potential to be severe and in very rare cases, even fatal (See our Risk Assessment protocols: http://www.chelseaforestschool.ca/index.php/en/risk-assessments/).

I understand that fires and open fire cooking are potential activities at Forest School and that my child may be  using tools such as knives and  bow saws after being well prepared and when closely supervised.   I understand that there are risks involved in these activities and that the safety rules will be explained to the children and that children will not be permitted to participate in these activities unless the safety procedures are followed and the teachers deem that the children are well prepared.
In case of an emergency I hereby consent to have my child taken to CHEO or nearest appropriate hospital by ambulance.  I understand that I (or my emergency contact) will be informed as quickly as possible.

I understand that payment can be made by cheque or in cash and is due on the day of the program. Cancellation less then 24 hours in advance will require full payment for the spot. Cancellation less than one week, but greater than 24 hours in advance will require 50% of the fee to be paid. There is no cancellation fee if your spot is cancelled greater than 7 days in advance. In the event that we do not receive enough registrations to run the program (min. 4), the program will be cancelled and any advance payment will be refunded. You will be notified of this decision a minimum of 48 hours in advance. *
Please read the confirmation notice that will appear on your screen after you hit submit. You will get an email early in the week of the program that you registered for, giving detailed information for the day. If you have not received this email and it is 48 hours before the program, PLEASE email or call us to confirm that we have your child on our list. *
