This form collects contact data for athletes interested in playing Water Polo, Swimming, or Diving.  This data is for communication by the coaching staff, team parents & club only.

It is extremely vital that all Parents fill out this form for all future communication for the upcoming Water Polo Season.  During the water polo season Parents will be asked to volunteer during polo tournaments and swim meets.  These are big fundraisers for the Aquatics Club, and we cannot function without raising money.  By working together, we all achieve more and can continue to provide a first class experience for our kids.
