Saint Charles Hall of Fame Nomination Form
Instructions
All information on this form must be completed and received no later than May 1st to be considered for the current selection cycle.
Please include as much of the information as you can:
1) Photocopies of newspaper clippings, articles, event programs, or other such material that
detail information of the nominee’s accomplishments. Please do not send originals as we are
unable to return these items.
2) A photograph of the nominee will be required should they be selected.
3) All information submitted shall be retained by the Hall of Fame Committee, and all action
necessary to the selection process shall remain confidential.

Process/Timelines
• Nominations will be accepted at any time throughout the year.
• Deadline for nominations in a given year is May 1st.
• The selection committee will meet after May 1st.
• Select and inform recipients by July 1st.
• Awards presentation will be at a football game or its own event in the fall. The first option will be the homecoming game, but the top priority is to have the recipients present.
