WAIVER AND RELEASE OF LIABILITY FORM
FOR THE TENNESSEE SINGING SCHOOL I am or will be attending the Tennessee Singing School held on the campus of Freed-Hardeman University. IN CONSIDERATION OF MY
REGISTRATION FOR THE 2024 SINGING SCHOOL (AND ASSOCIATED EVENTS
AND ACTIVITIES) CONDUCTED BY THE TENNESSEE SINGING SCHOOL AND
MY ATTENDANCE AT THE TENNESSEE SINGING SCHOOL AND ASSOCIATED EVENTS, I
HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THE SINGING
SCHOOL AND ANY AND ALL EVENTS AND ACTIVITES ASSOCIATED WITH THE
SINGING SCHOOL, including by way of example and not limitation, any risks that may
arise from negligence or carelessness on the part of the persons or entities being released,
from dangerous or defective equipment or property owned, maintained, or controlled by
them, my participation in any activities in connection with the Singing School which may
expose me to an infection, illness or disease of any type, or because of their possible liability
without fault. This assumption of the risk includes but is not limited to any personal injury,
death, disability, illness or disease which I may suffer, incur, or sustain while in attendance
at the Singing School.
I certify that I have no health-related reasons or problems which preclude my participation
in the Singing School and/or I have assessed the risks attendant to attendance and participation in
the Singing School given my age, physical condition, and health. I acknowledge that this Waiver
and Release of Liability Form will be used by the event holders, sponsors, and organizers of the
activity in connection with the Singing School in which I participate or attend, and that it will
govern my actions and responsibilities at the Singing School and associated events.
In consideration of my application and permitting me to attend and participate in the
Singing School and associated events and activities, I hereby take action for myself, my executors,
administrators, heirs, next of kin, successors, and assigns as follows:
(A)I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but
not limited to, liability arising from the negligence or fault of the Entities and/or persons
herein released, for my death, disease, infection, illness, disability, personal injury,
property damage, property theft, or actions of any kind which may hereafter occur to
me including my traveling to and from this activity, the following Entities and/or
persons: The Tennessee Singing School and its directors, officers, employees,
volunteers, representatives, agents, servants, and independent contractors, sponsors,
and volunteers; and
(B)I HEREBY INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE
the entities or persons mentioned in this paragraph and all of its subparts from any and
all liabilities or claims made as a result of attendance or participation in this activity
and/or associated events, whether caused by the negligence of release or otherwise
caused in connection with or by the Tennessee Singing School and its directors, officers, employees, volunteers, representatives, agents, servants, and independent
contractors, and sponsors, and volunteers;
(C)I HEREBY AGREE AND COVENANT to adhere to and follow all guidelines and
instructions for safety, good health, disease prevention, and accident prevention given
or promulgated by the Tennessee Singing School. I understand that any failure or
refusal to adhere to and follow such guidelines may result in me being barred from
further participation and attendance at the Singing School, which shall be in the sole
discretion of the Tennessee Singing School.
I acknowledge that the Tennessee Singing School and their directors, officers,
employees, volunteers, representatives, agents, servants, and independent contractors are NOT
responsible for the damages, errors, omissions, acts, or failures to act of any party or entity
released herein conducting a specific activity on their behalf.
I hereby consent to receive medical treatment which may be deemed advisable in the event
of injury, accident, and/or illness during the Singing School and any and all activities and events
in connection with the Singing School.
I additionally understand and agree that while participating in the Singing School and any
and all activities and events in connection therewith, I may be photographed. I agree to allow my
photo, video, or film likeness to be used for any legitimate purpose by the activity holders,
producers, sponsors, organizers, and assigns of the Tennessee Singing School.
The Accident Waiver and Release of Liability Form shall be construed broadly to provide
a release and waiver to the maximum extent under applicable law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND
ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A
CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
