Dear Stanbridge Families,
To streamline the annual data-collection process that accompanies the start of the new school year, we use an online method of completing annual school contact and waiver forms.
The data you submit through this form will be used to identify you and your student's contact information, as well as identify whom Stanbridge should contact in case we are unable to reach you in the event of an emergency.
THIS IS A REQUIRED FORM FOR ALL STANBRIDGE FAMILIES TO COMPLETE.The information you submit via this online form will then be imported into PDF versions of our standard office forms. After finishing this survey and our other online Back to School forms (Emergency Contacts Info and Waivers & Releases), we will send home a packet of required forms filled-out with your answers—you will simply need to review and acknowledge that the data is accurate.
Once you begin the survey, you cannot save your progress and return later, so below is an overview of the information we will be asking for so you can have the necessary documents and information on-hand when ready. We ask that you

The sections of this "2023-2024 Student Medical Information" form are as follows- (this page): A copy of your completed responses will be sent to the email address collected.
- Basic name and ID information.
- Special medical needs or health issues your child may have (allergy information, immunization records, physical limitations).
- A copy of your child's current immunization record.
- Your child's Covid vaccination status as of their first day of school this year.
- Over-the-counter medications your child is permitted to have administered at school.
- All medications, prescription or over-the-counter drugs, that your child takes.
- Medical and Dental insurance information (policy and group numbers; contact information for your child's primary doctor and dentist).
In addition, please note the appropriate childhood immunization schedule–Birth thru 6 Years Old OR 7-18 Years Old–and provide an official copy of your child's current record to Gladys in the Front Office.
