Massachusetts Youth Soccer Association's Annual General Meeting (AGM) - Attendee Questionnaire

Please complete this questionnaire so that Mass Youth Soccer can plan for the number of people attending the AGM. The sessions to be held include:

AGM Business - Year in review, reports, election of board director.

Stack Sports US Soccer Connect Training - Spring 2020 Adult Credential Administration using the current version of US Soccer Connect, Fall 2020 Adult Registration, Adult Credential management and player registration data uploading using the new version of US Soccer Connect, Club Connect/Blue Sombrero.

Zero Tolerance and Codes of Conduct - Interactive group discussion helping Mass Youth Soccer to develop a statewide Zero Tolerance Policy and Coaches, Parents and Player's Codes of Conduct.

This information will solely be used to estimate the number of attendees for meeting room and lunch planning purposes. This is not a formal confirmation as we know schedules and plans change for many reasons.
