Hello, TDs!
This is the form to register your games as an official event with the league as well as to get your event added to TourneyMachine. While we will do everything in our power to get events added expeditiously, but please try to submit your request AT LEAST two weeks before the scheduled start. This gives us time to setup your event, as well as your attending teams time to submit their attendance requests and update their rosters. This form should be filled out the Tournament Director that will serve as the event administrator on TourneyMachine. You will need the following information to complete this form:
1. Event Name
2. Event Date
3. Event Location (name and address of hosting facility)
4. Event Region
5. Event admin name (this MUST be a USQ-certified TD by start of event)
6. Event admin email (the email you’d like attending teams to contact you at)
7. Event admin phone number (this will remain confidential with USQ staff and only used in the event of an emergency related to your event)
