JHS 2020-2021 Schedule Request Form: 11th Grade

Please view the Scheduling Presentation posted on the JHS Website (jhs.lasallepsb.com), then use this form to request classes you would like to take for the 2020-2021 school year. Please make a selection for each question. This does not guarantee you will get in these classes, but we will do our best to accommodate your request. If you have failed a course, be sure to select to retake it. If the course you need is not listed, include it in the notes section.
A parent must sign by entering their name at the end of the form. Check your graduation requirements and TOPS requirements (if applicable) when selecting courses. Choose at least 8 classes total. Feel free to write a note at the provided space at the end of the form if there is a question or suggestion you would like to share. If you need additional help, email smanning@lasallepsb.com.
