Welcome to the Educator Dispositions Assessment Submission Form!

In order to complete this form, you will need each student's name, 700#, and program of study. A separate entry is required for each individual student you are scoring. Unless the indicator states otherwise, all dispositions and indicators require a score. There is an optional comment box at the bottom of each disposition page if you would like to provide evidence or an explanation of a given student's rating.

Explanation of ratings scale:

0-Needs Improvement: minimal evidence of understanding and commitment to the disposition
1-Developing: some evidence of understanding and commitment to the disposition
2-Meets Expectations: considerable evidence of understanding and commitment to the disposition
