Student Information and Parent Authorization Form

**This form will serve as authorization for all marching band related activities June 2022 thru December 2022. The Medical Documentation and Photo Release will serve as authorization throughout the academic school year June 2022 through May 2023**
PLEASE KEEP ALL INFO UP-TO-DATE AS IT WILL BE USED FOR THE ENTIRE SEASON

June 2022
June 21-22 9:00 am-3:00 pm (Marching Band, Winds, Percussion, Color Guard)
June 23 1:00 pm-6:00 pm (Marching Band, Winds, Percussion, Color Guard)

July 2022
July 18-22 (Marching Color Guard 9:00 am-5:00 pm)
(Percussion, Front Ensemble & Battery 9:00 am- 4:00 pm)

August 2022
August 1-5 8:00 am-5:00 pm (Marching Band Winds, Percussion, Color Guard)
August 8-12 9:00 am-5:00 pm (Marching Band Winds, Percussion, Color Guard)
August 15-18 9:00 am-5:00 pm (Marching Band Winds, Percussion, Color Guard)
August 19 9:00 am-5:00 pm (Parent Preview Day, Students will not leave but we will end rehearsal slightly early to prepare for the performance)

We are a team in every sense of the word. All rehearsals/practices listed above are necessary to the overall success of the band. Therefore, every effort should be made to attend all rehearsals.
