I understand that while I am in the United States of America, I must follow the laws of the United States of America, and the laws of any local areas (e.g. states and local municipalities) I am in.
As a student studying at Chatham University, I agree to follow the rules and expectations for Chatham University students as written in the Chatham University Student Handbook. I also agree to the following:

1. I will read and review all information sent out by OIA
2. I understand that if I am residing on campus, it is my responsibility to follow the guidelines of Residence Life especially in regards to move-in and move-out dates of my campus assigned housing. I understand that I cannot stay beyond the dates stipulated by Residence Life unless permitted by Residence Life.
3. I also understand that it is my responsibility to request transportation through OIA for airport pick up if I am a new student or find transport on my own if I am returning.

In addition, I understand that it is my responsibility to maintain my F or J student status. I confirm I will report any information that may affect my status in a timely manner to OIA. I will:

1. Report to OIA for check-in during the first week of the beginning of every semester (this form, in person, or by email);
2. Provide an accurate phone number where I can be reached within the United States.
3. Report changes in my name, address and/or phone number.
4. Report any significant changes in my financial information.
5. Report receiving a new passport and/or when I receive a new visa.
6. Come to OIA for advisement PRIOR to any drop below 12 credit hours/ 9 credit hours in my semester course load.
7. Come to OIA for advisement PRIOR to a change of major.
8. Come to OIA for advisement PRIOR to a transfer of institutions.
9. Come to OIA for advisement PRIOR to taking courses at another institution during the Chatham University academic
year.
10. Come to OIA for advisement PRIOR accepting or applying for any form of employment/internship on or off-campus.
11. Come to OIA for advisement PRIOR to traveling outside of the United States of America.
12. Report any other changes regarding myself that I feel would cause the records held by OIA to
reflect inaccurate information.

I also understand the necessary steps I need to take before traveling while I am on my F or J visa.

1. I must have a valid passport that will remain valid at least 6 months.
2. I must get a travel signature prior to leaving the U.S. (F students need a PDSO/DSO to sign and J students need a RO/ARO to sign)
3. If my visa has expired, then I must get a new visa prior to re-entry into the U.S. (You should apply for a new visa in your country of citizenship. It is more difficult to get a U.S. visa in a country where you are not a citizen. Please contact the U.S. consulate you wish to visit to ask for advice. If you are denied a visa, then you will not be able to re-enter the U.S.)

I understand that it is my responsibility to know and understand how to maintain my immigration status. I will make an appointment with OIA prior to any actions that may affect my immigration status.
