UW-Madison L&S Honors Program Distinguished Honors Faculty Nomination Form
This web form is to nominate faculty for the 2019 Distinguished Honors Faculty Award. This award was created to recognize outstanding work by faculty who teach Honors classes or work with Honors students in research. The survey will be open through Thursday, April 18, 2019 at midnight. If your nominee is selected, you will be notified and invited to attend a reception on May 3, 2019 at 4:00 p.m. with your faculty awardee.

In order to develop a stronger nomination for a faculty member, please consider sending the link to this nomination form to any friends in the Honors Program who may have had a similarly positive experience with the faculty nominee, and encourage them to submit a nomination as well.
