2. The form only needs to be filled out once, per child, for the 2023-24 school year.

6. A physical examination taken April 1 and thereafter is valid for the following two school years. A physical examination taken prior to April 1 is valid only for the remainder of that school year and the following school year. A current Physical Examination Form must be on file with the Athletic Department (email a photo to schererm@wawmsd.org or drop of the form in the athletic office). Here are some recommended clinics should you need to schedule a physical exam.

7. Athletic payments can be made online through your family's Skyward account. Student-athletes' payment will be posted on the account one week after final rosters are submitted to the athletic department. Payment must be made at the start of the season for your student-athlete to be eligible to compete.
