This self-assessment and scoring is for educational purposes to help you identify where your business is doing well and where there may be opportunities for improvement. This self-assessment is not intended to certify your business practices as COVID-safe or to certify your implementation plan.
Submissions are anonymous, and it should take about 15-20 minutes to complete. Scores are not recorded or used by Sauk County Public Health.
At the end of this self-assessment, your score will be calculated. Also provided at the end of the self-assessment is feedback based on your answers to provide you additional resources to help protect customers and staff.

If you scored between 0 and 50 points: There are opportunities for your business to implement additional protective measures to reduce the risk of spreading COVID to your customers and staff. Consider reviewing WEDC and SCPH Reopen Guidance and other resources provided in this self-assessment to learn how you can improve your business practices to help protect your customers and staff.
If you scored between 51 and 120 points: You are on the right track, but may want to consider additional changes. Review the resources provided in this self-assessment to continue your good work to protect your customers and staff.
If you scored over 121 points: Congratulations! You likely have the policies, procedures, and practices in place to help reduce the spread of COVID to your customers and staff. Please consider reviewing your answers and the provided resources for more ideas for improvement.
