I understand and assume all risks associated with my decision to have mail delivered on my behalf to 1414 Jefferson Ave. Maryville, TN 37804. I understand and will follow guidelines of pick-up times and here-by waive, release, discharge and hold J316 Outreach and partnering organizations harmless from any and all liability now or in the future, foreseen or unforeseen in the handling of my mail. I indemnify John 316 Outreach and its partners for any damages and loss of mail as a result of my participation in this mail service program. I understand that the mail can be picked up on Sundays at 5PM at 1414 Jefferson Ave.

This consent covers all information shared by you or by anyone that has the right to share information on your behalf and is relevant to the recipient's involvement in your care.
