Reporting a Concern (Including Bias Reports)

NOTE: Between March 28 and July 1, this form is monitored on Tuesdays and Fridays only. For urgent concerns, please contact your dean of students, principal, Human Resources, or other administrator.

This form is intended for community members to report concerns about conduct or situations in the Lab community. Reporting a concern is private and does not mean the individual who experienced the incident somehow loses control of the process. After a report is made, an appropriate administrator will assess the report and will reach out to the reporting person or the person affected by the behavior to discuss support and options.

You should consider using this form to report the following verbal, physical, or digital concerns:

- Bullying Behavior

- Bias Incidents, Discrimination, and Harassment

- Hateful Expression

- Physical Violence/ Threat to Others

- Repeated Concerning Behaviors

- Retaliation

- Defacement or graffiti on school property

- Other concerns that affect the learning environment or otherwise create a hostile environment for a member of the community

Note: Retaliation of any kind is strictly prohibited for any party involved with the reporting or resolution of a report under Lab policies.

Anonymity: Please note that the option to remain anonymous will be provided throughout the process. If a person reporting a concern asks to remain anonymous or declines to provide their name when submitting this report, then the school will consider how to proceed, taking into account their wishes and Lab’s commitment to provide a non-discriminatory environment, and students’ rights to have specific notice of any accusation. The school will conduct limited fact finding to better understand the context of the complaint. Please note, however, in some circumstances, a request for anonymity may mean an investigation cannot go forward, which means that the school may be unable to follow up on the reported concern.
