This form is to be completed for incoming 6th Grade Students to Ocoee Middle School. Please pick ONE slot from the time slots below. You must ONLY show up for the time slot you selected.

When reporting to the school, ONLY students & one parent should come to campus for the devices.
Student & parents must have a facial covering.
When arriving on campus, please report to the cafeteria.
Students should already have a password in mind (Criteria: 8 characters, no spaces, mix of letter and numbers, at least 1 capital letter)
