Dear Stanbridge Families,

To streamline the annual data-collection process that accompanies the start of the new school year, we are again using an online method of completing this year's annual school contact and waiver forms.

The data you submit through this form will be used to identify you and your student's contact information, as well as identify whom Stanbridge should contact in case we are unable to reach you in the event of an emergency.

THIS IS A REQUIRED FORM FOR ALL STANBRIDGE FAMILIES TO COMPLETE.

The information you submit via this online form will then be imported into PDF versions of our standard office forms. After finishing this survey and our other online Back to School forms (Emergency Contacts Form and Student Medical Information Form), we will email you a PDF packet of all required forms, fully-completed and filled-out—you will simply need to review and acknowledge that the data is accurate.

Once you begin the survey, you cannot save your progress and return later, so please ensure you have sufficient time to read through the content. As noted above, these are required forms for all students, and we ask that you PLEASE COMPLETE THIS ONLINE FORM ASAP. The sections of this "2023-2024 Stanbridge Academy Waivers and Releases" are as follows:

1. (this page): Basic name and ID information. A copy of your completed responses will be sent to the email address you provide.
2. Contact information for professionals your child may see (doctors, specialists, occupational therapists, and/or tutors); Permission to share educational information with specified school district.
3. Release of Information for Family Directory.
4. Permission and Release of Student Information for Stanbridge Counseling.
5. Permission to Exchange Information with School District.
6. Technology Acceptable Use Policy (requires student acknowledgment).
7. Parent-Student Handbook Agreement (requires student acknowledgment).
8. Parent/Guardian Driver Authorization (requires additional submission).
