This form needs to be completed by competitors and accompanying persons in Maroc's Balfour event to provide entry information and evidence of communication and agreement with additional precautions. It should be sent no later than midnight on Monday, 25th October.  Should someone develop Covid-19 symptoms after submission of the form, please do not attend the event.

Accompanying persons are people who attend the event and participate in part of it but are not competitors (e.g. helpers, people shadowing competitors, event officials)

This event is open to all. Please complete a form for each family member entering.

Entry fees for the event will be £6 for adults and £3 for Juniors/Students/adults on orange course or below, and £1 for a hired dibber. Payment will be taken on the day either using a debit/credit card (preferred) or cash. If you choose to pay by cash,  please bring the exact money. In the event that card payment does not work on the day, a link for online card payment will be sent to the email address submitted on this form, after the event.
