MHMS Transcript Request Form
Transcript request form for colleges, scholarships, personal, etc.  Mrs. Falise will submit your transcript within 5 business days during the academic school year.  Requests placed over the summer may take longer.

The cost per mailed transcript request is $2.00.  Payment MUST be made to Mrs. Falise before requests will be mailed out.  There is NO CHARGE for emailed transcript requests.  As a courtesy, no fee is charged if your parent works for Sumner County Schools.

For each transcript request the following will be mailed:  your official high school transcript, a copy of your test scores on file at MHMS* unless you "opt out", and school profile.

*Since some colleges are test optional when applying for the 2022-2023 academic year, I have made a option below for you to request no test scores to be sent.
