USE THIS FORM TO REPORT AND REFLECT on hours of service you completed in the Spring 2023 semester. You MAY NOT report service hours that occurred PRIOR to the start of the Spring 2023 semester.

You will receive one co-curricular credit for every two hours of service completed in a church, community, or campus context (up to 20 hours).

PLEASE NOTE: Co-curricular credit is not offered for paid work experience. This form is only for reporting voluntary service hours.

DEADLINE: This form must be submitted by11:59 p.m. on Wednesday, April 26, 2023. Credit will be issued once the Reference person you list in Section 3 has reviewed your reflections and approved your service hours.
