CDHS Community Involvement Hours Submission Form
This form can be used in place of the paper copy/written community hours sheets.  Once a student has completed a portion of their community hours they are to submit them to the Guidance Office using the form below.  This form can be submitted multiple times throughout a student's high school years.

Students require 40 hours as a component of the Ontario High School Diploma.  It is recommended to submit hours as they are completed. Do not wait until the total required hours are completed.

Information collected through this form will be verified. The hours will then be applied and recorded in the student's Ontario School Record (OSR).

Students are encouraged to check with Guidance Staff prior to completing an activity to ensure that they will be approved as appropriate community hours.
