  7. Copy of ITEP Exam Results  
  8. Copy of a Recent Bank Account (Showing Sufficient Funds to Pay for Tuition & Fees) 
  9. Skype Interview Required for Non-Agency Student
  10. Signed Statement of Faith
  11. Completed Student Essay
  12. $500 Non-Refundable Application Fee (Paid only upon Approval of Student Application & Supporting Documents)
