This form will be posted for voting online May 16, 2024 thru midnight May 21, 2024 for those
members who wish to vote online. Online voting will be the only option for this last round of votes for the CVHA's new logo.
All memberships will be checked in order to validate the votes. If a member is deemed to be in violation of
the CVHA membership/voting rules, their vote will not be counted. Also, if it is showed that a member
votes more than their allotted vote, their vote will be deemed invalid and will not count. Please keep in mind that the logos you are voting on may have to be slightly altered to ensure the logo is suitable for jerseys, banners, website etc.

As a reminder, the following are the criteria for membership/voting eligibility:
Membership
Membership in CVHA shall be open to all families regardless of current residence if such membership does
not violate any current MAHA requirements.
A member is:
* Any one person who is the parent, stepparent, foster parent, guardian or other adult identified as
being financially responsible for the account of a child properly registered with CVHA.
* Any current head coach, assistant coach, officer or director or anyone who holds an appointed
position with the CVHA, who does not fall under the previous option is considered a volunteer and,
therefore, a member.

Voting Eligibility
One parent, stepparent, foster parent, guardian, or person financially responsible for the account of a properly
registered child, providing the account is in good standing, and not under disciplinary review or probation shall
be granted one vote per child registered for the purpose of electing Board Members. However, each family
shall be allowed a maximum of two votes regardless of how many children are registered.
Coaches and/or managers who are listed on the MAHA roster and members of the Board of Directors are also
granted voting privileges, these privileges may not be used in addition to their voting privileges listed above.
Each member is allotted one vote regardless of whether that member holds multiple qualifying positions.

Voting by proxy shall not be permitted.

Any family that had or has a player signed up for fall 2023-24 and/or spring 2024 season is eligible to vote if they are in good standing. 

Here are some examples as who gets what votes.

Family 1 has 3 players in the association. 1 parent is a coach. Family 1 would be eligible for 2 votes. (as long as
their account is in good standing)
Family 2 has 2 players in the association. Only 1 parent is involved in paying, participating of behalf of both
players. Family 2 would be eligible for 2 votes. (as long as their account is in good standing)
Family 3 has 1 player in the association. 1 parent is a rostered Coach. Family 2 would be eligible for 2 votes ~ 1
for the player and 1 for the position held as rostered coach. (as long as their account is in good standing)
Family 4 has no players in the association. 1 person serves on the CVHA Board and/or is a rostered coach.
Family 4 would be eligible for 1 vote
