This is the volunteer registration form for community health fairs this Spring and Summer throughout the Los Angeles County. These events are a collaboration with the following organizations: Dodger Foundation, UCLA Health, and Gamma Tau-at-Large. We will be providing blood pressure screening, blood glucose checks, and nutrition education during this event. Please sign up for the day(s) that you are interested in and willing to commit to!
Set up time for all events (unless otherwise indicated): 8:30 AM
Event start time: 10:00 AM
Event end time: 1:00 PM
Items to bring on the day of the event: reusable water bottle, small snack (lunch is usually not provided), sunscreen/hat/sunglasses (all events will be outdoors)
Attire for nursing students/faculty: nursing school uniform, school ID/badge
Attire for registered nurses: scrubs of any color
