Mid Term/Final Evaluation for Field Instructors & Task Supervisors
Best practice is for you to review the questions and have a transparent conversation with your student. Students also have to self-assess on each competencies, ideally you would discuss their answers as well.

Task Supervisors and Field Instructors should discuss together and ideally meet with their student together and provide ONE evaluation. The email provided below will get a copy of the evaluation, Field Instructors can then forward the results to the task supervisor.
