声明及同意 Declaration and Agreement
1. 我证实所填具的个人资料属实并在此准许主办单位收集我的个人信息，并且可通过电话，短信，电邮等通讯方式随时联系我有关申请的进度，活动以及任何其它信息的更新。I hereby confirm that the information provided is correct and consent to organizer for collection and use of my personal data via telephone, SMS, email or any other means of communication that may not be notified to me from time to time, for information on application status, activities, and other updates.
2. 我理解并同意活动中的照片，音频或视频可以让主办单位进行宣传。并批准主办单位或主办单位所指定之人士使用实体拍摄的任何照片，录像或录音的权利。I understand and agree that photo, audio and/or video may be taken at our event for publicity purposes. You also waive any right to inspect or approve any photo, video, or audio recording taken by organizer or the person or entity designated to do so by organizer.
3. 我了解若在参与主办单位任何室内或室外活动 / 项目中造成任何伤病或财产 / 个人财物上之损失，主办单位均不会为其附上责任。I understand that organizer will not be liable for any injuries or loss of properties / personal belongings incurred during my participation in any of their indoor and outdoor activities / programmes.
