THIS FORM IS ONLY FOR EXTENDED CHAPEL EXEMPTIONS, NOT FOR EXEMPTIONS FOR INDIVIDUAL DAYS.  If you are isolated for COVID reasons, you are automatically allowed to stream chapel and get credit for it that way.  If you have another reason you have had to miss an individual chapel service, simply count it as one of your four excused absences.  If there is a reason you believe that you should be granted an individual exemption for a service or two, please email me directly at Michael.Jordan@houghton.edu.

Please note that chapel exemptions are not automatically granted.  We will try to respond to these applications within a week.  If your request is approved, you'll need to stream chapel, keep a log of the chapels you attended and turn the log in at the end of the semester.    

A couple guidelines:
--We have discontinued regular/automatic COVID exemptions this semester.  If you have a particular medical concern that makes you particularly concerned about COVID exposure, these will be evaluated on a case-by-case basis.
--Class/educational exemptions will allow you to stream  either once or twice per week, depending on the situation
--Work exemptions will onlly allow you to stream once per week--students may only work during one of the required chapel hours.
--Part-time and commuter students are exempt from the chapel requirement altogether, and will not be required to stream (though of course you are welcome to!)
