In order to ensure the safety of your child during the school year, we will need a list of adults authorized to meet your child at the bus stop. The driver will not release Kindergarten /1st graders, if there is not an authorized adult there to meet them with the exception of First Graders whose parents complete SECTION 3. Only First Graders may be released to walk home alone if parent completes the waiver section below. In the event they cannot be released, they will be returned to the school. THIS IS FOR STUDENTS ON BIG BUSSES ONLY. Please complete one form per student for Kindergarten and 1st grade students being transported by BIG BUS ONLY.

The list of authorized adults will be shared with the bus driver and the school. Any changes through out the the year must be resubmitted on this form which will be on our website for your access.
