After filling this form, please wait to receive your Item/s Pick-up confirmation email.

After receiving your Item/s Pick-up confirmation email, retrieve the item/s from the indicated Grab ‘n Go Keyboard Check-Out Area.

Please note that you can only request one Keyboard (61 key Keyboard, 88 key Keyboard or 88 Key MIDI Controller).

Use the provided case to transport the Keyboard (we highly encourage transporting the Keyboard with the assistance of someone else, especially when going up/down the steps). Please let the Library Staff know if you need assistance.

Please return your item/s to the Music Liaison Desk by the Due Date.

When you're ready to return your item/s please fill the Portable Keyboard Check-In Form (https://forms.gle/Fku51uRZj7oEwedK7).

You are liable for any overdue equipment and any damage the equipment sustains while checked out. This also includes smaller accessories within a kit. Fines related to repair, replacement, and late returns will be charged to the patron’s student account and may affect eligibility for future rentals
