University of Waikato Esports Club membership (Hamilton) - 2022
All University of Waikato students, staff and recent alumni are invited to join our club free of charge. Fill in this form and come and see us at the Esports Arena at 164 Hillcrest Road. We're open 10-4pm Monday to Thursday during teaching time (not open during teaching recess, exam periods or holidays).

Terms and conditions
- The membership is valid until 31 October 2022.  The membership is free. Membership provides you with access to the OMEN ESPORT ARENA arena during opening hours (refer Facebook University of Waikato Esports). and up to 10 hours per week on the PCs. Time on the PCs does not carryover. A UOW Esport Arena Assistant is present during these opening hours.
- The esport OMEN by HP PCs are managed through ggLeap.
- Within the OMEN ESPORT ARENA personal belongings are the owner’s responsibility.
- You are bound by the discipline regulations of the University of Waikato.
- Time limit: 2 hours per session. 10 hours per week. R18 games with gross violence and drug use will not be provided.
- Please respect other users of the OMEN ESPORT ARENA.  We encourage good personal hygiene, bad language or behavior is not tolerated.  You may be asked to leave the esport arena.
- Any injuries or incidents in the facility must be reported to reception.
- Personal belongings are the owners responsibility
- If differences and disputes arise between members of the University community, it is expected that they will attempt to resolve such conflicts reasonably and co-operatively. Formal procedures should be used only where other methods of resolving conflicts have been ineffective or would be inappropriate.
- Any serious breaches of these Terms and Conditions or persistent violations of any aspect of these Terms and Conditions may result in membership with UOW Esports being terminated.  
- I agree to this personal information being provided to the University of Waikato.  University of Waikato Esports has obligations under the Privacy Act 1993 regarding its collection, use, storage and disclosure of personal information of an individual.
- I agree to any photos or video taken of me while in the OMEN Esports Arena being potentially used for promotional purposes.
