Welcome! Requesting accommodations and registering with our office is a multi-step and interactive process. Please complete this form to begin this process. This form will provide our office with important information related to your disability and request for accommodations. All information submitted in this form goes directly to the office of Accessibility Services and is handled confidentially.

Once this form is submitted, the office of Accessibility Services will review your information and email you to explain the next steps in the accommodations request process.  These steps are also outlined on our webpage under "Prospective Students" followed by "Registering with Accessibility Services".
